Bold Body Peace partners with Fay Nutrition, LLC (“Fay”) for insurance verification, billing, and secure telehealth administration.
Bold Body Peace remains responsible for self-pay billing and payment collection for clients who do not use insurance or whose sessions fall outside insurance coverage.
Please review these policies to understand your rights and responsibilities when receiving nutrition counseling and coaching services through Bold Body Peace and Fay.
Insurance & Billing
For clients using insurance, Fay Nutrition manages benefit verification, claim submission, and payment collection.
While Fay makes every effort to verify eligibility and estimate costs, your final coverage and out-of-pocket amounts are determined by your insurance plan. It is your responsibility to confirm coverage, deductibles, copays, coinsurance, and any required preauthorization or referral before your visit.
If a claim is denied, missing, or delayed, you are responsible for the outstanding balance up to the provider’s self-pay rate.
For out-of-network visits, you will be billed at the self-pay rate and may request an invoice (superbill) to submit to your insurer for potential reimbursement. Claims cannot be resubmitted for out-of-network visits.
Current in-network plans include: Aetna, Cigna, Golden Rule, Optum, Oxford, Select Health, United Healthcare, United Medical Resources (UMR)

Self-Pay Billing (Bold Body Peace)
For clients not using insurance, or for services not covered by insurance, payment is handled directly by Bold Body Peace.
Rates: All self-pay sessions are billed at Bold Body Peace’s standard private-pay rates.
Payment: Payment is due at the time of service via major credit/debit card or HSA/FSA card.
Receipts: Superbills can be provided for potential out-of-network reimbursement.

Financial Policy
At booking, clients must provide valid credit or debit card information (and, if applicable, insurance details). Fay securely stores this information for insurance billing purposes. For self-pay clients, payment information is collected and managed directly by Bold Body Peace.
Charges that fail due to outdated or invalid information remain your responsibility.
Payment is due after each session. Fay will charge your card for insurance patient responsibility balances; Bold Body Peace will process payment directly for self-pay sessions.
All transactions are processed through secure, HIPAA-compliant systems.
Cancellations and Missed Appointments
Appointments not canceled at least 24 business hours in advance, and appointments where you arrive more than 50 percent into the allotted time, may be billed at up to the full self-pay rate. Cancellation and no-show fees cannot be billed to insurance. A $60 no-show fee will be billed to payment form on file.
Two consecutive missed or late-canceled sessions may result in discharge from care.
Residence and Telehealth Consent
By scheduling an appointment, you confirm you are a resident of the state selected when searching for your provider.
Services may be delivered virtually through secure video, audio, or text-based platforms. You may withdraw telehealth consent in writing at any time. Alternatives may include in-person sessions when available, though these cannot be guaranteed.
All privacy laws regarding medical records apply to telehealth.
At the start of each session, you’ll be asked to confirm your physical location to assist emergency services if needed.
Good Faith Estimate (No Surprises Act)
If you are self-pay or not using insurance, you are entitled to a Good Faith Estimate (GFE) of expected charges.
You will receive a written estimate at least one business day before your scheduled session and may request one at any time.
If your bill is $400 or more above your estimate, you may dispute the charge.

Notice of Privacy Practices
Fay Nutrition and Bold Body Peace may use and disclose your health information for treatment, payment, and healthcare operations as permitted by law.
Other uses require your written authorization unless a specific exception applies (e.g., emergencies, abuse reporting, judicial proceedings, or health oversight).
Your Rights
Inspect and Copy: Request access to your records; reasonable copy fees may apply.
Amend: Request corrections in writing; some requests may be denied under law.
Accounting of Disclosures: Request a list of certain disclosures for up to six years after your last service.
Restrictions: Request limits on disclosures. If you pay in full out of pocket, you may request that we not share that information with your insurer.
Confidential Communications: Request that we contact you in specific ways or at specific locations.
Complaints: File privacy complaints with Fay Nutrition or the U.S. Department of Health and Human Services.
Email, Text, and Notifications
Email and text messaging are used for scheduling, reminders, and billing updates. These are convenience tools only and not appropriate for emergencies. If you are experiencing an emergency, call 911.
Authorized staff may read or process email and SMS communication to support care or billing operations, and such messages may become part of your record.
By providing your phone number, you consent to automated calls or texts about appointments, insurance, or payment.
If you opt in to marketing messages, you may receive promotional texts such as reminders, offers, or newsletters. Message and data rates may apply. Reply STOP to opt out or HELP for assistance.
Scope of Practice
Bold Body Peace provides nutrition counseling and coaching within the licensed scope of a Registered Dietitian Nutritionist (RDN).
This work supports behavioral change and wellbeing but is not a replacement for medical treatment or psychotherapy. When needed, referrals will be provided.
Privacy and Security
All telehealth sessions, scheduling, and billing are managed through secure, HIPAA-compliant systems.
Insurance-related data is managed by Fay Nutrition. Self-pay clients’ payment and communication data are managed by Bold Body Peace.
We comply with all federal and state privacy regulations and do not share information without consent, except as required by law.
Calendar Integration
If your provider uses Fay’s calendar synchronization, the platform may read and create events in your connected Google account.
Fay’s use of Google API data follows the Google API Services User Data Policy, including Limited Use provisions.
Emergencies
If you are experiencing a medical or mental-health emergency, call 911 immediately.
Telehealth, email, and text systems are not monitored for urgent or emergency communication.
